右丙亚胺对表柔比星所致心脏毒性防治作用的观察

摘要:
目的观察右丙亚胺(Dexrazoxane,DEX)对表柔比星(Epirubicin,EPI)所致心脏毒性的防治作用。方法选择化疗方案中含EPI的60 例经病理学确诊的乳腺癌、胃癌与恶性淋巴瘤的患者,按病种选择相应方案化疗2 个周期后,随机分为对照组和观察组,每组各30 例,对照组继续原方案化疗4 个周期,观察组则在原化疗方案上加用DEX(DEX颐EPI = 10∶1),2 组患者均按要求完成4 个化疗周期,观察心电图、左心室射血分数(LVEF),肌钙蛋白I(cTnI)定性检测的变化。结果2 组心电图异常差异、LVEF 变化从第4 周期开始均有统计学意义(P <0. 05 ~ P <0. 01);2 组的cTnI 定性检测无统计学差异。结论右丙亚胺对表柔比星所致心脏毒性的发生有一定的防治作用。
 Objective To observe the preventive and therapeutic effects of dexrazoxane (Dexrazoxane, DEX) against
the cardiac toxicity induced by epirubicin (Epirubicin, EPI). Methods A total of 60 patients who were diagnosed by pathological examination in breast cancer, gastric cancer or malignant lymphoma, and having the chemotherapy regimens with EPI were selected. Corresponding chemotherapy was chosen according to the cases. All patients were treated for 2 treatment cycles, and then were randomly divided into control group and observation group, 30 cases in each. The control group continued the original chemotherapy for 4 treatment cycles, while the observation group used the original chemotherapy combining with DEX (DEX∶EPI =10∶1). Patients in both groups were treated for 4 treatment cycles. The changes in electro-cardiogram, left ventricular ejection fraction ( LVEF) and cardiac troponin I ( cTnI) qualitative detection were observed. Results The 2 groups of electrocardiogram abnormal differences and LVEF changes were statistically significant (P < 0. 05 or P < 0. 01) since the fourth cycle, while the cTnI qualitative detections were no statistical difference. Conclusion Dexrazoxane have certain protective effects against the cardiac toxicity induced by epirubicin.
