摘要：

目的探讨药学服务在2型糖尿病合并高血压患者中的应用价值。方法选取我院收治的2型糖尿病合并高血压患者资料64例，随机分为对照组和观察组。对照组31例按常规2型糖尿病合并高血压症治疗原则给予药物治疗；观察组33例由临床药师在常规治疗的基础上提供药学服务。观察比较随访前后2组体质量指数（BMI）、空腹血糖（FBG）、糖化血红蛋白（HbA1c）、收缩压（SBP）、舒张压（DBP）及用药依从性（Morisky评分）等指标的变化，记录不良反应发生率、用药差错率及满意率等情况。结果治疗后，观察组的各项指标（BMI、FBG、HbA1c、SBP、DBP、Morisky）与治疗前比较，差异均有统计学意义（P<0.05，P<0.01）；对照组的HbA1c、Morisky评分与治疗前比较，差异无统计学意义（P>0.05）。观察组的不良反应发生率和用药差错率低于对照组，而满意率高于对照组。结论在2型糖尿病合并高血压症的治疗过程中，临床药师提供药学服务能够提高治疗效果，减少药物不良反应及用药差错的发生，并能提高患者的用药依从性和满意率。

OBJECTIVE To explore the value of pharmaceutical care on patients with type 2 diabetes mellitus(T2DM) and hypertension. METHODS 64 patients with T2DM and hypertension were randomly divided into control and observation group. 31 patients in control group received standard routine treatment, whereas 33 patients in observation group received pharmaceutical care besides standard routine treatment. The changes of body mass index(BMI), fasting blood glucose(FPG), glycosylated hemoglobin(HbA1c), systolic blood pressure(SBP), diastolic blood pressure(DBP) and medication adherence(Morisky) before and after treatment,adverse reactions(ADR), medication errors and satisfaction were observed and compared between the 2 groups. RESULTS After 3 months treatment, the indicators(BMI、FBG、HbA1c、SBP、DBP、Morisky) of observation group were significantly improved; while the levels of HbA1c and Morisky in control group had no significant difference before and after treatment. ADR and medication errors of observation group were lower than those in control group, observation group's satisfaction was higher than the control group. CONCLUSION Pharmaceutical care of clinical pharmacists could elevate the clinical therapeutic efficacy of T2DM and hypertension, reduce the ADR and medication errors, and improve the medication adherence and satisfaction.
