胰岛素用于不同孕周妊娠糖尿病患者对新生儿脑损伤的影响

摘要:
目的 探讨胰岛素用于不同孕周妊娠糖尿病(GDM)患者对新生儿脑损伤的影响。方法 选取2015年5月~2017年3月在某院接受治疗的GDM患者103例,入选产妇均给予饮食控制及适量运动。其中有53例患者在孕32周前开始给予胰岛素治疗(早期组),50例患者在孕周大于或等于32周时开始胰岛素治疗(晚期组)。新生儿出生1 h后抽取股静脉血,检测血清MBP、S100B及NSE的水平,超声检测新生儿颅脑损伤的情况。结果 晚期组脑损伤发生率(18.00%)明显高于早期组(3.77%),差异有统计学意义(P<0.05);早期组新生儿血清中MBP、NSE、S100B水平均显著低于晚期组,差异有统计学意义(P<0.05);两组低血糖发生率比较,差异无统计学意义(P>0.05)。结论 对于GDM患者,及早给予胰岛素治疗能减少新生儿发生脑损伤,且安全性高。
OBJECTIVE To investigate the effects of insulin on neonatal brain damage in patients with gestational diabetes mellitus(GDM) at different gestational weeks. METHODS From May 2015 to March 2017,103 patients with GDM were treated in a hospital,all women were treated with diet control and regular exercise. Among these,53 patients were given insulin therapy before 32 weeks of gestation(early group),and 50 patients were given insulin after 32 weeks of gestation (late group).The serum levels of MBP,S100B and NSE were detected after 1 hour when the baby was born. The brain damage of newborns was examined by ultrasonography. RESULTS The incidence of brain injury in the late stage group(18%) was significantly higher than that in the early stage group(3.77%),with significant difference(P<0.05); the serum levels of MBP,NSE and S100B in the early group were significantly lower than those in the late group,with significant difference(P<0.05); the incidence of hypoglycemia was compared between the two groups,the difference was not statistically significant (P>0.05). CONCLUSION For patients with GDM,early treatment with insulin can reduce the incidence of neonatal brain injury with a good safety profile.
